Health/Dental Insurance: Our health insurance benefit is available to all Sapphire Lutheran Homes employees who work in a regularly scheduled position at least 30 hours per week.  We offer both medical and dental plans.  Sapphire Lutheran Homes contributes towards the employee’s portion of the medical premium.  You may add dependents to the policy if you desire. Enrollment forms must be submitted to Administrator and St. John’s Lutheran Ministries Human Resources within 30 days of hire. The coverage becomes effective the first of the month after 90 days of employment.  Pre-tax premium deduction is optional. Please note: Failure to meet the enrollment deadline will result in disqualification from participating until the Annual Group Re-Enrollment. In addition, pre-existing conditions would become applicable if you could not submit a Certificate of Credible Coverage from your previous Insurance carrier.*Enrollment in pre-tax premiums and opportunities to change annual elections are administered in accordance with the Department of Treasury Section 125 guidelines. 

 

Life Insurance:
For any employee that enrolls in the health insurance plan, a $10,000 group term life insurance policy is automatically activated.  The contract amount is available for employees only. 

 

Flexible Spending Accounts: AFLAC: Supplemental insurance through AFLAC is available to all Sapphire employees regardless of hours worked.  Offered are Accident, Cancer, Hospital, Intensive Care, Vision, Dental and Short Term Disability Policies.  Sapphire does not contribute towards these policies.  You may enroll after 90 days of continued employment.  Pre-Tax deduction is optional.  *Enrollment in pre-tax premiums and opportunities to change annual elections are administered in accordance with the Department of Treasury Section 125 guidelines.  
 
Vacation: An employee who works 30 hours per week or more is eligible for vacation after one year of continued employment.  Vacation time is paid at employee’s current rate of pay and must be requested 30 days in advance. 
 

Sick Time: An employee who works 30 hours per week or more is eligible for sick pay after 90 days of employment.  An employee can accumulate a maximum of 160 hours.  After accumulating the maximum, the following year an employee would be paid a bonus for half of the amount exceeding the 160 hours at their current rate of pay.  The year end bonus for half of the amount of accumulated sick leave in excess of 160 hours may be paid upon termination or retirement.  Sick leave will not be paid on the first day of any given illness, until 160 hours have been accumulated.
